Payment Details

I hereby grant full permission to any and all of the above parties to use any photographs, videos, motion pictures, website images, recordings or any other record of this event taken of me &/or my minor child for any legitimate purpose.

Waiver of Liability

In consideration for participating in or volunteering for activities (each, an "Activity") for the benefit of or organized by the Virginia Down Syndrome Association (the "Releasee"), I, on behalf of myself and anyone who may claim on my behalf, including, but not limited to, my heirs and personal representatives, hereby unconditionally and irrevocably release, forever discharge, and hold harmless the Releasee, its affiliates, and successors and assigns, and each of the foregoing's officers, directors, employees, volunteers, representatives and agents, from any and all claims, liabilities, losses, expenses, and obligations of any nature, which I may have arising out of or relating to my participating in or volunteering for the Activity, and I agree to indemnify the Releasee for any and all liabilities, losses, obligations, and expenses the Releasee incurs related to my participating in or volunteering for the Activity. I understand that participating in or volunteering for the Activity involves inherent risks, including, but not limited to, risk of physical injury to person or property or death, and I hereby assume all such risks. I further understand that I may be photographed or otherwise video recorded while participating in or volunteering for the Activity, and I hereby agree to allow my photograph or other film likeness to be used for any legitimate purpose by the Releasee.

I hereby agree that I have reached the age of 18 years or have caused my parent or legal guardian to execute this Waiver of Liability below; that I have carefully read all of the foregoing; and that I fully understand its contents.
